Yamaha Marine Protection Plus Program
Please note, this program is available only for new Yamaha boats equipped with new Yamaha motors, trailers are included, if shown on the invoice. To obtain an application, select the following Yamaha Marine Protection Plus Program link.
The client is to complete, print and sign the application form and faxed with a copy of the customer's invoice to David St. Martin, DKC Insurance Group, fax number 905-475-0466 and a similar copy should be faxed to Yamaha Program Coordinator, Eagle Underwriting Group Inc, fax number 905-455-5298.
N.B. Without a copy of the invoice, insurance coverage cannot be bound.
These documents become material application documentation for Underwriter's files.

Coverage starts only after the fully completed and signed application is received by Eagle Underwriting Group Inc. A policy will be sent to the client from the DKS Insurance Group office shortly therafter.
All applications are subject to Underwriters' approval and restrictions.
